Worker's Compensation

The Bio-Back is covered by most state Worker's Compensation programs, and generally requires pre authorization. Medolutions will work with you to determine your state's specific reimbursement and coverage requirements.

Medicare

Remember that you must have received notification that you have been approved as a DMEPOS supplier before you can dispense (and be paid by Medicare) for a Bio-Back. Once you have your DMEPOS supplier number, Medolutions will provide the forms that need to be completed for each Medicare order. There are very specific requirements for Medicare billing, but we will be happy to work with you to successfully implement your program if you are a new Medicare B supplier.

If you do NOT participate with Medicare, one of our participating Medicare Suppliers can fulfill orders for your Medicare patients. Please contact Customer Service at 888-978-7999 Ext 103 for additional information.

Insurance

For any insurance plan, we always recommend that you contact the plan to determine that specific plan's coverage and reimbursement for HCPCS code L0627. You should also determine if a deductible applies as some plans have additional DME deductibles.

Medicaid/Medicaid MCO

The Bio-Back is covered by most state Medicaid programs, though reimbursements and prior-authorization requirements vary significantly by state. For states with MCO/HMO type programs, each payor will need to be contacted to determine specific coverage requirements.
